第４話は、マンションの一室で、20代の若い女性の遺体が発見されることからスタートします。女性はウェブデザイナーの坂井亜衣です。
亜衣の姉の坂井美優からマンションの大家に、妹と連絡が取れないため様子を見に行ってほしいという電話があったんです。マンションの大家が警察を伴い部屋に入ると、ベッドに横たわって亡くなっている亜衣が発見されたんです。
野毛山署強行犯係の刑事たちとともに現場に入る朝顔（上野樹里さん）です。遺体を調べると、亜衣は窒息したようで、首にはうっすらとひも状のもので絞められた痕があったんです。何者かに首を絞められて殺された可能性が考えられました。
しかし、大家が警察とともに亜衣の部屋に入ろうとした時は、確かに扉には鍵が掛かっていたため、これは密室殺人なのではないか、と刑事たちは疑い始めるんです・・・。

高梨臨さんは、第４話では、亜衣の姉である坂井美優を演じてくれます。
介護士として多忙な日々を送っている美優は、真面目な人柄で妹思いの優しい女性なんです。もともとは姉妹の仲は非常に良かったのですが、最近は、とある理由で、姉妹の間には距離ができていました。
そんな中で亜衣が亡くなったことで、警察からの聞き取りを受ける美優。そこで美優は、妹を殺した犯人に心当たりがあると言い出したんです・・・。

『監察医 朝顔』の、優しく流れるような時間がとても好きなので、出演のオファーをいただいた時はとても嬉（うれ）しかったです。
しかし、台本を読んだ時に、なんとも救われない気持ちになりました。
この役をいただくには、すごく重いものを背負わないといけないなというプレッシャーが大きかったです。
上野樹里さんとは初共演となりますが、撮影現場はいかがでしたでしょうか？
上野さんとは初めてお会いしましたが、言葉や、表情ひとつひとつが全て素敵で、自分の訴えている感情や、うちに秘めている気持ちまでも汲み取ってくれているように感じました。
お話の内容や役柄的にも、現場であまり多くは、お話はできませんでしたが、キャストの皆さん、スタッフさん達のチームワークが良く、とても居心地の良い素敵な現場でした
